•    Supervises: Respiratory Therapists, Pulmonary/Respiratory Nurses, Bronchoscopy Technologists; provides clinical supervision to junior doctors for respiratory care
Primary purpose:
Provide specialist respiratory care and clinical consultation across OPD, ICU, ward and perioperative settings: diagnose and manage relevant acute and chronic pulmonary conditions, perform airway procedures (bronchoscopy), manage ventilator-dependent patients, and support perioperative respiratory optimization for cardiac surgical patients.

Key responsibilities:
1.    Clinical Care (OPD/ward/ICU/perioperative): Provide specialist assessment, diagnosis and management of acute and chronic respiratory conditions for outpatients, inpatients and perioperative cardiac patients; respond to emergency respiratory consults.
2.    Ventilation strategy & weaning: Lead ventilator management for post-operative and ICU patients (including non-invasive support and lung-protective strategies); authorise and oversee ventilator/weaning protocols and daily ventilator rounds.
3.    Airway procedures & bronchoscopies: Perform and supervise paediatric flexible bronchoscopies (diagnostic and basic interventional), ensure safe sedation/anesthesia coordination, maintain procedural and sampling records.
4.    ECMO & advanced support coordination: Initiate/escalate ECMO or extracorporeal support when indicated and coordinate bedside cannulation and management with perfusion, surgery and ICU teams per local policy.
5.    Rapid response & emergency airway care: Lead management of respiratory deterioration (severe ARDS, airway obstruction, ventilator failure), direct emergency airway/rescue measures and activation/escalation pathways.
6.    Training & supervision: Train, supervise and assess respiratory therapists, nurses and bronchoscopy staff; develop local competency frameworks, run simulation drills and provide bedside teaching.
7.    Investigations & perioperative optimisation: Interpret ABGs, chest imaging and other respiratory tests; advise on extubation readiness, chest physiotherapy, aspiration prevention and perioperative respiratory plans.
8.    Protocols, quality & infection control: Develop/implement clinical protocols (ventilation bundles, sedation, bronchoscopy reprocessing, IPC); participate in M&M, CQI activities and monitor KPIs (e.g., ventilator days, VAP/ventilator complication rates, bronchoscopy safety metrics).
9.    Procurement & maintenance advice: Advise on specifications and selection of respiratory equipment (ventilators, bronchoscopes, humidifiers, suction systems) and coordinate preventive maintenance with Biomedical Engineering.
10.    Documentation, MDT participation & on-call: Maintain accurate clinical records and HMIS entries in English; attend multidisciplinary meetings; participate in the hospital on-call rota for respiratory emergencies (including nights/weekends/holidays) and be available for urgent ECMO/airway activations per policy.

Required Qualifications:
Mandatory:
•    Medical degree (MBBS/MD or equivalent).
•    Postgraduate specialty qualification in Pulmonology / Respiratory Medicine or Pediatric Pulmonology (MD/FCPS/FRCP or equivalent).
•    Current medical registration with Afghan Medical Council (AMC) or eligibility; valid medical license.
Preferred:
•    Fellowship or formal training in Pediatric Pulmonology.
•    Formal critical-care / dual Pulmonary & Critical Care training.
•    Bronchoscopy and ventilator competency certificates and a procedural logbook (bronchoscopies and major ventilator cases; last 24 months) if available.
Minimum experience
•    Minimum 4-5 years post-specialty clinical experience managing respiratory diseases; for pediatric-weighted post, at least 2-3 years experience in pediatric pulmonology or equivalent supervised practice.
•    Documented recent (last 24 months) procedural experience in bronchoscopy and management of ventilator-dependent patients in ICU.
