•    Supervises: CICU medical team, senior ICU nurses (shift in-charge), respiratory therapists, ECMO/perfusion liaison and redeploy able clinical staff on shift 

Primary purpose (one sentence)
Provide senior consultant-level intensive care for post-operative and medical pediatric cardiac patients; lead the CICU clinical team during shifts, manage advanced organ support (including ECMO when available), drive clinical governance and training, and ensure safe, timely, family-centered critical care consistent with ARCS standards.________________________________________
Key responsibilities
1.    Provide senior clinical leadership for the CICU: assess, stabilize and manage neonates, infants and children after cardiac surgery and medical cardiac admissions, and set daily priorities for the unit.
2.    Direct invasive hemodynamic and fluid management: order/interpret invasive monitoring (arterial/central lines), titrate vasoactive/inotropic therapy, manage fluid strategy and coordinate post-bypass circulatory goals with surgery and perfusion.
3.    Lead respiratory support and weaning: manage ventilator strategies for neonates/children, supervise advanced ventilation modes, trouble-shoot respiratory deterioration and decide extubation readiness.
4.    Initiate and manage extracorporeal support: lead ECMO initiation, bedside management and weaning when in service, coordinate cannulation and ongoing care with perfusion, surgery and ICU teams.
5.    Supervise multidisciplinary rounds, handovers and escalation plans: ensure clear, documented management plans, timely escalation and safe transfer decisions (to theatre, cath lab or other units).
6.    Lead emergency responses and surgical escalation: direct management of tamponade, massive bleeding, arrest, life-threatening arrhythmias and make decisions on re-exploration or urgent interventions.
7.    Train, supervise and credential staff: teach and assess trainees, nurses and allied staff in advanced procedures (central/arterial lines, chest drains), POCUS, ECMO drills and critical-care competencies.
8.    Ensure medication and device safety: oversee weight-based dosing, anticoagulation protocols (post-op/ECMO), blood-product management, maintain device/ECMO/line logbooks and ensure accurate HMIS/ICU registry entries.
9.    Drive quality and IPC: lead M&M reviews, root-cause analyses, audits (device/line bundles, VAP/CLABSI metrics), implement corrective actions and ensure adherence to IPC and device-care bundles.
10.    Operational readiness and coordination: maintain ICU readiness for surgical lists (equipment, blood availability, escalation pathways), advise on procurement/maintenance of ICU equipment, participate in retrievals/transfer decisions and cover senior on-call responsibilities as rostered.

Mandatory qualifications (must-have) & preferred
Mandatory:
•    Medical degree (MBBS / MD or equivalent).
•    Postgraduate specialty qualification in Pediatrics or Anaesthesia plus an accredited fellowship/qualification in Pediatric Critical Care Medicine / Pediatric Intensive Care (or equivalent recognized qualification).
•    Current medical registration/license (Afghan Medical Council) or eligibility to register.
•    Current PALS/APLS and NRP certification.
Preferred / desirable:
•    Fellowship or formal training in Pediatric Cardiac Critical Care or substantial documented experience in a pediatric cardiac ICU.
•    ECMO credentialing, POCUS competency, advanced ventilator training and experience with neonatal/infant extracorporeal support.
Minimum experience
•    Minimum 3 years post-specialty clinical experience overall with at least 1-2 years substantial experience in pediatric critical care, and ≥2 years focused experience in pediatric cardiac ICU or equivalent neonatal/infant cardiac intensive care.
•    For foreign candidates: minimum 5 years at senior level in recognized paediatric critical care units, with documented ECMO/complex post-op cardiac ICU exposure preferred.
